Please verify that all scans are clear and taken on a plain background. If scans are not clear, they will be rejected and will delay your application process. 

Born in Québec:

- Québec Birth Certificate (long version)

- Valid Medicare card or driver's license as proof of Québec residency

Born in Canada (Outside of Québec):

- Birth Certificate with parents' names 

- Valid Medicare card or driver's license as proof of Québec residency

Born outside of Canada:

ONE of the following documents:

- Canadian Citizenship card or document (both sides)

- Permanent Resident card (both sides)

- Refugee - CAQ & CSQ documents 

AND all of the following documents: 

- Passport 

- Valid Medicare card or driver's license as proof of Québec residency

Payment

Drop in for current students / by appointment:

We accept payment by debit or credit (Visa or MasterCard). If you are registering in person, you will receive your books and materials immediately.      

By phone: 

We accept payment by credit (Visa or MasterCard) - we will contact you once we receive your application. If you are registering by mail, please expect 2 weeks for registration, evaluation and shipment of books and materials.
